Seminarian Application Part 2
Listed below are two forms that constitute the second portion of the application. Please print these forms, using the back arrow to return to this page and retrieve each of the next forms. When the forms are completed, please send them to our office or upload them below, along with a copy of your background check (see note below). If you fill them out on your computer, we have found it works best to save the file with a different name, fill it out, and then save again.
Once these forms are received in the IPF office you will be mailed a third part of the application process: the Creighton University 'Immunization Requirement' form. This form must be completed and returned by February 1, 2019. If you are accepted, a registration hold can be placed on your account by Creighton Student Health if the form is not returned or the dates do not comply with the guidelines. The registration hold will affect your ability to attend the summer program. In some cases a physician's signature may be required. Please do not put off completing the 'Immunization Requirement' form once you receive it.
Even though an application has been submitted online, this does not guarantee placement in the program. To be considered for admission, all three parts of the application must be received by February 1, 2019. All applications will be reviewed on a rolling admission basis. Once the enrollment maximum is exceeded, only those with completed applications on file will be considered actively on a waiting list. Application status will be sent out mid February 2019. *Note: Together with your application materials please send a copy of your background check which was completed when you applied to your diocese. If you do not have a copy of the background check, please ask your diocese to forward one to our office.
