Listed below is a form that constitutes the second portion of the application.  Please print the form, using the back arrow to return to this page. When the form is completed, please send it to our office or upload it below, along with a copy of your background check. If you fill it out on your computer, we have found it works best to save the file with a different name, fill it out, and then save again.

Part II Documents

  • Background check - Contact your Vocations Office for a copy of your background check which was completed when you applied to your diocese

Once the Part II documents have been received, the IPF office will send information with the instructions to enroll into the Creighton system. In addition to the online application, Creighton University will require all IPF students to be compliant with the University Immunizations Policy. Immunization requirements can be found here. Be prepared to provide the immunization requirements as part of the IPF/Creighton University enrollment process. Immunization requirements must be met prior to the February 1st deadline to be considered for acceptance.

Even though an application has been submitted online, this does not guarantee placement in the program. To be considered for admission, all parts of the application process must be completed by February 1, 2022. This includes being compliant with the immunization requirements. Please note that the application process can take up to 2-3 weeks to complete from start to finish. All applications will be reviewed on a rolling admission basis. Once the enrollment maximum is exceeded, only those with completed applications on file will be considered active on a waiting list. Application status emails will be sent out mid February 2022.
